The pathogenesis of the lesion is believed to be a complex cascade of events characterized initially by antigen antibody complexing at the basement membrane zone, followed by complement activation and ultimate elaboration of chemotactic factors, followed by attraction of PMNs to the area and release of proteolytic enzymes that ultimately dissolve or cleave the basement membrane zone, usually at the lamina lucida level Eversole, 1994 ; . Pemphigoid management is somewhat more complicated than that of pemphigus vulgaris, due to the extreme variability of steroid responsiveness. In addition to systemic and or topical steroids, clinicians may consider using antibiotics such as tetracycline or one of its congeners, as well as niacinamide, scrupulous oral hygiene, and other immunosuppressants. Personal as well as published experience in the use of antibiotics, particularly tetracycline in a non-antimicrobial mode, has proved to be gratifying and effective in many cases. In addition to the anticollagenase effects of the tetracycline antibiotics, it is believed that they help inhibit PMN chemotaxis and random PMN migration. These agents also may inhibit complement-induced inflammatory responses to basement membrane antigen-antibody complexes and may help maintain the cohesion of the epithelial connective tissue junction. Foster and colleagues 1992 ; have demonstrated that, with the use of systemic immunosuppressive agents in a carefully controlled regimen, prolonged periods of remission were achieved in the management of ocular cicatricial pemphigoid, even following withdrawal of the drugs.
